Standard is the name of the manufactuer. i believe they make all the mopar, FBO, and Car Quest (lx101) boxes. the Proform blue box is made somewhere in Asia i believe.which is the manufacturer?

Can you tell me how you determined that? It looks nothing like the LX-101 photo at Summit:

The heat sink is very distinctive
your last link is NOT a "Standard" brand name box. I hope we are not confusing you. The Summit/ Rock Auto box is a good quality box, and is FOUR pin NOT five, and will plug right into either a 4 pin (resistor) harness or a 2 pin (resistor) harness.
"Standard Motor Products" is a brand name, many times just called "Standard." "Blue Streak" is there REALLY high quality stuff, but in the last few years, some of their "Blue Streak" stuff has been eliminated.
In other words a 4 pin box will work in any Mopar ECU installation and will work with either 2 or 4 pin ballast resistor
The 5 pin box is an OLDER design, and MUST have the older 4 pin ballast
