Emission sticker question

Can anyone identify the car, year etc. inner fender emmision sticker 44 28998 goes on?

In Classic Industries catalog it list it for a 1968 manual trans with 225 and 383 4v and 1969 auto trans with a 225 and a 318 2v. Check them out because I could be getting it wrong. Good luck
