Decoding fender plate for Dave

Of cause I’ll give it a try, give me a couple of days and I’ll send it back as an attachment. I should be able to get about 90% of it figured out. Yes it a Lynch tag, in most cases they repeated a lot on the information from the bottom of the tag, but used different numbers.

Related to the plate, last line first 4 letters/numbers what are they.
If you take the plate off, place in a de-rust solution, then with a light brushing or steel wool clean it. Light wash it then lightly spray with a clear finish, the plate will look a lot better.
