This is a brand new valve from Year One #DTB7 - BLK246. It is listed to be for 1969-1970 Mopar B-body Brake Distribution Block for models with Disc Brakes. Mine RR has drum brakes and got the wrong one. About the cheapest you will find this is $80.

4 wheel disc?

I do not know. I know only what Year one and others post about that. I would assume front disc and rear drum as was the common factory set up.
