Just to follow up for anyone that might run into this in the future, new fork from Brewers showed up the retainer being bent/worn was 100% the issue, comparison photos below
Short story - talked to Dan at Brewers and he feels confident the retaining spring on the clutch fork is worn/bad, he said it should hold the fork to it and mine does not, see photo below of fork on stock bell with stock pivot with bellhousing sitting face down on a table. New fork is on order...
What doesnt look right exactly? The bracket I made corrected the bracket on the lakewood bell that had the ballstud way out of whack, I made a jig on the stock bell that I then transfered to the lakewood to make the bracket and locate the ball stud in the same location.

No experience with it yet but I have this tank with the 340 lph pump ready to go into my 69 super bee with a 600hp 505. Planning to do a mix of -8an and 1/2" hardline for feed/return and I got the Aeromotive dual fuel pressure regulator (13220). They rate the setup for 750hp for NA carb engines.
Looking for a fan shroud (2949130) and the matching brackets/hardware. Will probably just end up buying them new but figured I'd ask if anyone has some they are looking to get rid of.
