I went looked at a D 200 with a factory 440 this week. It has 58000 miles and all original. It has a big time shudder if you give it hard acceleration from a dead stop. It also made noise when I used reverse. When driving normal it shifts fine with no issues. The owner is not mechanically inclined at all. It has been doing this since before he bought it a year and a half ago. The previous owner disclosed it. It has a Dana so I doubt that is the problem. A friend said it may have a 2 piece drive shaft and could be the carrier bearing and or the u joints. Another buddy thinks it sounds like the torque converter. What do you guy think?

I have a 74 W300 ClubCab, exact same issue, truck has intermediate shaft between trans and transfer case., so rebuilt that....aaarrrrggghh. Found low reverse band had gotten loose. Dashboard shaker, lol. In reverse, there is more pressure applied, which is why they don't usually shudder. But that is where I would start. check the servo if the band adjustment is ok. But the fact that it only happens in first, eliminates quite a few other issues that could cause it. Good luck.

This is a regular cab 2 wheel drive. Do you think it has a carrier bearing? I'm gonna go look at it again Sat.

Regular cab should have the one piece drive shaft...ie, no carrier bearing. Check the trans mount for one. The Dana could be worn out but even then, they usually won't shudder. If the mount is good, then I'd look at the trans next but when they shift ok driving normal, they usually are ok when getting down on them but check the fluid level. Do the easy checks first. Try to look at the stick in good light and see if there's anything that sparkles at ya and smell the fluid. I've see nice red fluid that smelled like crap.
