Mystery document

[69 Charger]

I visited my Charger's restoration shop today, and the owner presented me with a document that he found when he removed the headliner. Looks like a final inspection sheet filled out by the paint inspector. Anyone ever seen one of these? I'm wondering what the codes mean that are circled. Sorry if the images are small. I have all of the original documents for the car, plus a 156-page logbook of maintenance and trips that I've kept since Day One, and this one will be a part of the collection. It has been quietly resting in the headliner for 53 years now, waiting for me to find it.
