Delivery inspection form?

Well I started tearing apart my interior and under the carpet I found a form. I thought it was a broadcast sheet but it didn't look like one. The form was badly corroded and really hard to make anything out. The only word on the top of the form I could make out was "Department" and then the form was filled with a breakdown of the panels of the car. Kind of like what you see on a car rental agreement that the guy inspecting the car will circle the various panels if there is damage present. Is this a pre-delivery inspection sheet or something? I wish it would have been in better condition so I could salvage it but oh well. At least I did find a little bit of the broadcast sheet hanging off the front bench seat.

If it's a heavy paper, I found one in my '70 GTX as well. I've only talked to one other person who had seen one, and many years ago, G Govier wanted me to fax him a copy of what I had cause he said he had never seen one.

In talking to many people about it, and a thread on another board with the other person who had found one, it's pretty well thought that what that is is a final inspection of the vehicle before it departs or on final assembly. Mine was in pretty decent shape, and it showed that the car had the wrong intake & carburetion, and a non working turn signal. Those problems were corrected and noted on the form.

Heck, take a pic of it and post it up. But for sure, hang on to it.

I took a pic of it so I can post it but you can't make out really anything on it. I did manage to peel a few chunks of it away from the jute pad but the majority of it was destroyed or had so much water damage there was no more ink on the paper. I haven't looked at the pic I took yet so I'll take a look and see if it's even worth putting it up on here.
