If it's got a replaceable plastic or brass fill valve seat, it could be cracked or chipped.

They can be a beotch to get out and they do make a special tool.

I've also seen hard water particles get stuck or build up on the seat.

The seal (flapper my name for it) gets hard and doesn't seal. The seat below that the flapper sits on could be damaged. That was bad on a toilet I had replaced toilet for other reasons. Weeping inlet shut valve.
That's exactly what's happening with the throne I'm sitting on currently.
 
Flapper rubber (or rubber gasket) can warp especially if it's been dry for a while, like if you left the tank empty overnight while you worked on the other parts.
 
All good points, with the exception of the tank to bowl gasket. If that fails water leaks onto the floor when flushed. It’s usually the flush valve leaking water slowly in the bowl. You can sometimes hear the fill valve topping off the tank water, or a constant hissing from older style valves. Sometimes the fill valve won’t shut off completely. Look for a overly high water level with water draining into the overflow tube
 
Original Poster has not said if he replaced the flapper or not. (At least, I have not seen that...)
 
Mentioned earlier the toilet was winning 3 to 0 and it’s now 4 to ahh 0. At first, I figured it was the gasket beneath the flusher valve, it was bad and easy fix. Didn’t have success after all the crap (how applicable huh?) I’ve done, the sucker STILL leaks into the bowl. This has been a problem longer than I thought too, just became more noticeable.

The flusher valve (on the left) doesn’t seat well enough to stop the trickle-leakage into the bowl. Only way I get it to stop is pressing firmly down on this. This is with the new gasket I put in. Reseated it and same result. Put in a new inlet valve that isn’t a good match to the original as I thought it should be, the design of the top is higher than the old one so dealing with it to allow the tank top to sit right.

Anyway, the hassle looks to be the flush valve seating. Going to try another gasket since it’s easy and also ordered a new valve assembly thru Amazon. If this doesn’t work, we’ve been considering a new toilet anyway. Could be this one is cursed

Next step in the battle - had no leakage or bubbling there thinking it was ok. So far: Toilet is up 3 wins to my zero. lol
Usually it’s so slow you don’t notice it. If you shut the supply off, the water will slowly go down. Hope this helps.

Did the OP look at the washer/disk that seals the fill valve when the float presses on it?

In my experience that's one of the most common issues.

Have to believe I’m in one of those weird auras of time where nothing seems to go well. I mean fixing a toilet is elementary barring something that’s out of the ordinary. It’s the flush valve not seating and still won’t seal despite a new gasket. Going to replace the flusher assembly when it arrives as stores I've been to doesn’t stock it.

Two weeks ago hit a deer in the wife’s car taking it out of commission and still in the body shop. Last Saturday, we were up at our daughter’s home 220 miles away. My wife used my Dakota running errands and gets rear-ended. Fortunately, she wasn’t hurt and it’s still drivable, but my truck I spent four months restoing 2 years ago...most of that needs repairing/replacing.

Lol, a member said bad things come in three’s, if I'm lucky, hope the toilet counts as the 3rd.

If ya see the flush valve on the left in my photo, that gets raised up by the lever connected to that loop at the top allowing tank water to make the flush. When letting go if it – it floats back down to a rubber gasket that’s supposed to seal off the tank. No flapper setup here. Something is amiss as despite a new gasket, it doesn’t seal unless I press down on it. Let it go and the trickle of water restarts into the bowl. That part seems fine, no cracks, warpage, etc. New gasket seems to insert perfectly. MO, must be something goofed with the assembly beneath it, all I can guess. Took the guts out and tank off as they needed serious cleaning since having to work on it, not intending to replace them, a CLR bath.

New toilets are cheaper than the grief your getting from that rig!
 
Maybe a small crack on the plastic flush valve itself, letting water into it
 
I can hear them if they don't completely stop running.

That flush valve looks oddball to me.

I'd replace that and get guaranteed matching gaskets at the same time.

They do make adjustable height fill valves.

With the tank off. Take the big nut off the bottom, remove the fill tube. You will see a rubber seal that seats between the bottom of the tank and the fill tube. It looks like you have hard water, (not as hard as Arizona). Replace that o-ring, gasket, whatever it is and it will fix it. Make sure there’s no mineral deposits on the bottom of the tank. It will leak out, but the seal between the tank and bowl, keeps it from leaking where you could see it. I know that’s what the problem is. If you’re getting complete new guts for it, that will be included. Take care and I hope you don’t get tanked!
