13 inches of vacuum at idle, needle shakes slightly. Idle is noticeably rough. I tried the timing every which way. Heads were done by a reputable shop. Cam is stock, rest of valve train is new Rock Auto parts. Cam came with engine and best i could find out was it was stock for a 383 2 barrel..... No vacuum leaks i can find...
Intake manifold gasket?
It has one very noisy lifter on start up. Any Thoughts

'Stock for a 383' should definitely be more, sounds like a compression test is in order. That should point you in the right direction before you take stuff apart. My money's on a vacuum leak under the valley pan. Especially if your shop milled the heads a little. Hopefully not a valve/cam/lifter problem...
 
I assume that your valve train and heads are all torqued down correctly ?
 
Any specs on the cam? How do you know it's a stock cam?

Maybe a compression test and infrared thermometer at each exhaust runner

Might have resurfaced heads. Im leaning in your direction

Did your engine overheat or why did you rebuild the heads and change the cam ? Heads redone, valve train new, new stock cam, I assume new head gaskets and all new for tuning the engine; you post before a brake problem , don't you have a vacuum leak at the brake booster maybe? Does the pedal brake sound like a cat when you step in it , just a thought
 
You could try and pull a plug wire off each cylinder and replace while it's running. But insulate yourself from the car as much as possible.
Could be one or two cylinders not firing.
New plugs I assume? ( I had an Autolite plug that was bad right out the box a few years back)
The rest of the ignition system is good?
Can you hear a miss in the exhaust?
